Complete and print a fill-able application from the Department of Transitional Assistance website.

Note

You can only view, complete, and print fill-able PDF forms.

• You cannot save your completed PDF form on your computer.
• You can save the completed Word Files on your computer.

Once you complete the application, make sure you print it, sign it, and mail or fax it to your local DTA office.
