Get the Application for Individuals to Pay the Filing Fee in Installments form from the US Bankruptcy Court.

In the top box put your name and your spouse’s if you are both filing.

Skip the box that asks for the District. Put “Mass.” in the box that asks for your state.

The clerk will give you the Case No. when you file your Bankruptcy Petition.

Most of this form is easy to fill out. Some parts are not so easy to understand. It is important to understand the question before you answer it.

The Application for Individual to Pay the Filing Fee in Installments is based on your income, expenses, and household size.  The fee for a chapter 7 is $335.  You must tell the court how you want to pay the fee.  You must pay it in 4 payments or less.. The total amount has to be paid within 120 days from the day you file your case.

For example

$110 paid on the day you file your case December 1, 2016

$75 to be paid on January 1, 2017

$75 to be paid on February 1, 2017

$75 to be paid on March 1, 2017

Sign and date this form. 

Use today’s date.

It does not need to be the date you actually file your bankruptcy.

If you are filing jointly, your spouse must also sign and date the form.
