To make a Certificate of Service, write on a piece of paper the following information. Then sign it, date it, and mail it to the following address with copies of what you sent the Immigration Court or the Board of Immigration Appeals. Also see Appendix A.

I certify that on this date I served a copy of the attached materials on the Department of Homeland Security by causing them to be placed in first class mail, postage prepaid, addressed as follows:
